Identification process in Utah

The Health Department of Utah requires applicants, once they have completed and signed the application form, to prove their identity by including a photocopy of their ID.

Acceptable document of identification are:

Provide one (1) of the following identification documents:

  • Photo driver's license
  • State Id
  • Us/Foreign Passport
  • Military Identification Card
  • School Identification Card
  • Private Company Employment Identification card
  • Permanent Resident Card (green card)
  • U.S. Certificate of Naturalization
  • U.S. Certificate of Citizenship
  • Concealed Handgun License
  • Employment Authorization Document (EAD)
OR

Provide two (2) of the following identification documents:

  • Medical/Medicare insurance card
  • Signed Social Security card
  • Veterans Affairs card
  • Bank Statement
  • Photo driver's license expired not more than five years
  • State Id expired not more than five years
  • Selective Service Card
  • Vehicle registration
  • Utility bills
  • Paycheck stub with imprinted information
  • W-2
  • Voter Registration
  • Certified deeds or title of property

Please note: If you can provide documentation of legal interest, or if you are holding a Power of Attorney from the person on record, you are eligible to request the vital record for that person.

How Long Will Birth Certificate Delivery Take?

It will take about 1 to 3 business weeks to get your birth certificate.The state needs ample time to process the number of inquiries they receive. Delays may vary depending on the volume being processed.

Exception and Date Limit?

If you were born before 1905 you will need to contact the Archive department in order to access to your original records.
