Identification process in Texas

The Health Department of Texas requires applicants, once they have completed the application form, to prove their identity by including a photocopy of their ID and notarizing the “application form”.This process is very common and used by many states. Acceptable document of identification are:

Provide one (1) of the following identification documents:

  • Photo driver's license
  • State Id
  • Military Identification Card
  • Offender Identification card
  • Employment Authorization Document (EAD)
  • Permanent Resident Card (green card)
  • Us/Foreign Passport
  • Concealed Handgun License
OR

Provide two (2) of the following identification documents:

  • Current student identification
  • Signed Social Security card
  • Medical/Medicare insurance card
  • Veterans Affairs card
  • Birth certificate
  • Private Company Employment Identification card
  • Form I-94 - accompanied by the applicant's Visa
  • Foreign Identification with identifiable photo of applicant

How Long Will Birth Certificate Delivery Take?

It will take about 4 to 8 business weeks to get your birth certificate.The state needs ample time to process the number of inquiries they receive. Delays may vary depending on the volume being processed.
