Identification process in Oregon

The Health Department of Oregon requires applicants, once they have completed and signed the application form, to prove their identity by including a photocopy of their ID.

Acceptable document of identification are:

Provide one (1) of the following identification documents:

  • Photo driver's license
  • State Id
  • Us/Foreign Passport
  • Current student identification
OR

Provide two (2) of the following identification documents:

  • Utility bills
  • Medical/car insurance card
  • Employment Authorization Document (EAD)
  • Concealed Handgun License
  • Vehicle registration
  • Paycheck stub with imprinted information
  • W-2
  • Voter Registration
  • Bank Statement

Please note: If you can provide documentation of legal interest, or if you are holding a Power of Attorney from the person on record, you are eligible to request the vital record for that person.

How Long Will Birth Certificate Delivery Take?

It will take about 3 to 5 business weeks to get your birth certificate.The state needs ample time to process the number of inquiries they receive. Delays may vary depending on the volume being processed.
