Identification process in Michigan

The Health Department of Michigan requires applicants, once they have completed and signed the application form, to prove their identity by including a photocopy of their ID.

Acceptable document of identification are:

Provide one (1) of the following identification documents:

  • Photo driver's license
  • State Id
  • Us/Foreign Passport
OR

Provide two (2) of the following identification documents:

  • Marriage certificate
  • Divorce certificate
  • Paycheck stub with imprinted information
  • W-2
  • Bank Statement
  • Voter Registration
  • baptismal certificate
  • Military Identification Card
  • Utility bills
  • Medical/Medicare insurance card
  • Lease/rental agreement
  • Offender Identification card

Who may apply for birth or death certificate?

Please see the following list of person authorized to order a vital record:

Birth
  • Person on records
  • Parent
  • Legal guardian
Death
  • Parent
  • Child
  • Sibling
  • Spouse
  • Legal guardian

Please note: If you can provide documentation of legal interest, or if you are holding a Power of Attorney from the person on record, you are eligible to request the vital record for that person.

How Long Will Birth Certificate Delivery Take?

It will take about 3 to 5 business weeks to get your birth certificate.The state needs ample time to process the number of inquiries they receive. Delays may vary depending on the volume being processed.

It will take about 3 to 5 business weeks to get your death certificate.The state needs ample time to process the number of inquiries they receive. Delays may vary depending on the volume being processed.
