Identification process in Maryland

The Health Department of Maryland requires applicants, once they have completed and signed the application form, to prove their identity by including a photocopy of their ID.

Acceptable document of identification are:

Provide one (1) of the following identification documents:

  • Photo driver's license
  • State Id
  • Us/Foreign Passport
OR

Provide two (2) of the following identification documents:

  • Utility bills
  • Vehicle registration
  • Signed Social Security card
  • Paycheck stub with imprinted information
  • Bank Statement
  • Income tax return
  • Employment Authorization Document (EAD)
  • Lease/rental agreement

You must also include a return envelope.

Who may apply for birth or death certificate?

Please see the following list of person authorized to order a vital record:

Birth
  • Person on records
  • Parent
  • Legal guardian
Death
  • Parent
  • Child
  • Sibling
  • Spouse
  • Legal guardian

Please note: If you can provide documentation of legal interest, or if you are holding a Power of Attorney from the person on record, you are eligible to request the vital record for that person.

How Long Will Birth Certificate Delivery Take?

It will take about 2 to 4 business weeks to get your birth certificate.The state needs ample time to process the number of inquiries they receive. Delays may vary depending on the volume being processed.
