Identification process in Illinois

The Health Department of Illinois requires applicants, once they have completed and signed the application form, to prove their identity by including a photocopy of their ID.

Acceptable document of identification are:

Provide one (1) of the following identification documents:

  • Photo driver's license
  • State Id
OR

Provide two (2) of the following identification documents:

  • Military Identification Card
  • Voter Registration
  • Us/Foreign Passport
  • Vehicle registration
  • Bank Statement
  • Medical/car insurance card

Who may apply for birth or death certificate?

Please see the following list of person authorized to order a vital record:

Birth
  • Person on records
  • Parent
  • Legal guardian
Death
  • Parent
  • Child
  • Sibling
  • Spouse
  • Legal guardian

Please note: If you can provide documentation of legal interest, or if you are holding a Power of Attorney from the person on record, you are eligible to request the vital record for that person.

How Long Will Birth Certificate Delivery Take?

It will take about 10 to 15 business days to get your birth certificate.The state needs ample time to process the number of inquiries they receive. Delays may vary depending on the volume being processed.

How Long Will Death Certificate Delivery Take?

It will take about 10 to 20 business days to get your death certificate.The state needs ample time to process the number of inquiries they receive. Delays may vary depending on the volume being processed.
