Identification process in Florida

The Health Department of Florida requires applicants, once they have completed and signed the application form, to prove their identity by including a photocopy of their ID.

Acceptable document of identification are:

Provide one (1) of the following identification documents:

  • Photo driver's license
  • State Id
  • Us/Foreign Passport
  • Military Identification Card

  • Grand child
  • Legal guardian

Please note: If you can provide documentation of legal interest, or if you are holding a Power of Attorney from the person on record, you are eligible to request the vital record for that person.

How Long Will Birth Certificate Delivery Take?

It will take about 5 to 10 business days to get your birth certificate.The state needs ample time to process the number of inquiries they receive. Delays may vary depending on the volume being processed.

It will take about 5 to 10 business days to get your death certificate.The state needs ample time to process the number of inquiries they receive. Delays may vary depending on the volume being processed.
